Qualifications:

Education & Work Experience:

Work Experience Required: 
 
·         Bachelor’s Degree in a health-related field of counseling, psychology, social work, or other behavioral health fields desired
·         Three to five years’ experience in the health care or social work field
 
 
Skills/Knowledge Required:

·         Ability to follow up and document the progress made by the patients.
·         Provide health education for individuals and groups.
·         Develop educational materials like articles, fact sheets, and public service announcements for the community. 
·         Set up meetings with providers and patients as needed and informing all necessary parties of the processes involved in each patients’ treatment and the plans for ongoing treatment. 
·         Keep updated records of each client, collect, and analyze data to evaluate all treatment and recovery programs, and continuously improve patient offerings.
 
 
 
Essential Functions & Responsibilities:

  • Assessing needs: Evaluating patients' strengths and needs, and determining the most appropriate treatment options 
  • Developing treatment plans: Collaborating with counselors and providers to create a treatment plan for each patient 
  • Linking patients to resources: Connecting patients with the services and support they need, including medical staff. 
  • Educating patients: Providing behavioral health education to patients and groups, and developing educational materials 
  • Monitoring progress: Following up with patients to document their progress and evaluate treatment and recovery programs. 
  • Advocating for patients: Speaking up for patients who may not be able to do so for themselves. 
  • Providing support: Assisting patients with medication education, illness education, and medication management 
  • Continual assessment of patients’ strengths, needs, support, and goals.
  • Participating in community events and networking opportunities
  • Collaboratively complete comprehensive and individualized care plan, crisis plan, PRAPARE screening tool, other assessment tools within 30 days of services, and every 6 months thereafter. Goals include both physical health and behavioral health, as well as any additional domains as applicable.
  • Assist with referral and linkage to internal and community resources to ensure patients are successfully connected and receiving support needed.
  • Provide side by side support to patients in addressing barriers and learning new skills. Advocate for patients whenever necessary.
  • Assist patients with attending appointments (Clinic, Therapy, IHC/PCP) by providing appointment reminders, collaboratively addressing barriers to attendance, and arranging transportation when needed.
  • Provide education to patients regarding gaps in care and collaboratively develop action plans to close gaps and manage health proactively.
  • Ensure efficient, timely, and coordinated transition of patients when needed within the clinic and the community support systems.
  • Meet productivity standards as indicated in the Policies and Procedures.
  • Meet with LCSW for monthly supervision and meetings as scheduled.
  • Come prepared to supervision meetings with questions, caseload information, and solutions for challenges.
  • Respond to urgent/crisis consumer needs with assistance and guidance from supervisors, crisis services, and clinic staff by developing and implementing plans and interventions.
  • Develop and maintain positive and professional relationships with community networking partners.
  • Ensure timely and appropriate documentation as required by the agency.
  • Collaborating with the Medical Team for integrated care for the patient.

Physical Demands

·         Requires a full range of body motion including manual and finger dexterity and hand-eye coordination.
·         It requires standing and walking for extensive periods of time. Occasionally lifts and carry items weighing twenty-five pounds or less.
·         Requires corrective vision and hearing to normal range. May require working under occasional stressful conditions and/or irregular hours.
 

Typical Working Conditions:
·         Infrequent exposure to communicable diseases, toxic substances, medical preparations, and other conditions common to a clinic environment.
